General Forms and Resources

Advantage Student Information Sheet (PDF): Successful completion of the Brenau Occupational Therapy Program requires a satisfactory background check and drug screening done through Advantage Students. This sheet provides information on package details, placing an order, tracking results, printing completed reports, and sharing results with schools and/or fieldwork sites.

Medical Records Information (PDF): This resource provides helpful tips on how to begin an adequate system of documenting your personal health records that may be required by Brenau and/or the fieldwork site, and offers instruction on how to submit necessary records to the site.

Brenau OT Student Health Form (PDF): Students may use this form to record their immunization history, physical exam results, and TB skin test results. (Note: any official physician documentation will suffice when submitting documentation).

Brenau University 2023-24 COI: Fieldwork Students are covered by Brenau’s blanket malpractice insurance policy.

Fieldwork

General Information: Fieldwork is a valuable and vital component of the Occupational Therapy Program here at Brenau. Its purpose is to complement the curriculum by offering opportunities for the application of evidence-based interventions, clinical reasoning skills, and overall professional behaviors. Fieldwork may occur in a variety of settings including medical, community-based, or educational facilities. Through fieldwork, skills are gained that transition the individual from student to practitioner.

Brenau Fieldwork Packets are distributed by the university at the beginning of the semester to the student and Fieldwork Educator. It helps orient each party to the progression of fieldwork, as well as what is to be expected in the particular fieldwork level and area. Some of these forms are required to be reviewed and returned, while others are simply references to help everyone get the most out of their fieldwork experience!

How Can I Be Successful in Fieldwork?

The transition from the classroom to fieldwork is a scary time for many students. First and foremost, rest assured that your program has provided you with the knowledge and skills required to be successful in the clinical world. Level I Fieldwork Packet

Level I Arrival Form (PDF): The fieldwork arrival form is filled out and returned the first day of fieldwork. It is used to track student term, experience area, site location, fieldwork educator contact info and credentials, and to ensure that all Brenau and site objectives for fieldwork are clear and met.  This information is also used to schedule the student and fieldwork educator in E*Value.

Fieldwork Objectives (Level I) (PDF): The objectives provide both Fieldwork Educators and students with clear expectations for the skills they should master over the course of their level I experience.

Fieldwork Evaluation (Level I) (PDF): The Level I Evaluation is completed by the Fieldwork Educator at the midterm and final points of the semester. The evaluation is on a pass/fail basis and must be reviewed with the student:Note: This document is a sample as the Level I Evaluations can now be completed online through the EValue system.

Student Evaluation of Fieldwork Experiences (SEFWE) (PDF): The SEFWE is the student’s opportunity to provide feedback about their fieldwork experience to his/her Fieldwork Educator and Brenau faculty through scaled questions and open ended comments. The SEFWE is discussed between the Fieldwork Educator and student. Note: This document is a sample as the SEFWE can now be completed online through the EValue system.

Fieldwork Educator Evaluation of the Fieldwork Experience (PDF): The Fieldwork Educator’s evaluation of fieldwork is the educator’s opportunity to provide the university with valuable feedback regarding the efficiency of procedures and the overall value of taking Brenau Students.

Level II Fieldwork Packet

AOTA Fieldwork Objectives (Level II) (PDF): The objectives provide both Fieldwork Educators and students with clear expectations for the skills they should master over the course of their level II experience. Fieldwork Educators may include additional objectives specific to their site.

AOTA Level II Evaluation: The Level II Fieldwork Evaluation is a Fieldwork Educator’s evaluation of student performance at the midterm and final. Note: This document is a sample as all Level II Evaluations must be submitted in original, hard-copy format based on AOTA requirements.

Level II SEFWE (PDF): The SEFWE is the student’s opportunity to provide feedback about their fieldwork experience to his/her Fieldwork Educator and Brenau faculty through scaled questions and open ended comments. The SEFWE is discussed between the Fieldwork Educator and student.Note: This document is a sample as the SEFWE can now be completed online through the EValue system.

Fieldwork Educator Evaluation of the Fieldwork Experience (PDF): The Fieldwork Educator’s evaluation of fieldwork is the educator’s opportunity to provide the university with valuable feedback regarding the efficiency of procedures and the overall value of taking Brenau Students.

Weekly Conference Form (Level II) (PDF): The Weekly Conference form is a document students may use to guide communication with their Fieldwork Educators to reflect on strengths and potential areas of growth, while coming up with new goals as skills progress.

Student Treatment Plan Template (Level II) (PDF): Students can use this template as they begin to independently assess and treatment plan for actual clients during level II fieldwork.

Suggested Progression of Expectations (Level II): These suggestions are a general week-by-week outline of the expectations for the student regarding workload, treatment planning, assessment completion, and increased independence in intervention.
